News Update

IT system changes in response to new data privacy laws will change the way that patient records can be shared between different GP practices. 

 If you are a person who has not permitted the sharing of your patient record with other GP practices, this will prevent you from accessing week day evening and weekend appointments that are held at our Weekend Plus Hubs. SMS messages have been sent to these individuals. If you have received the text message, please reply ‘Y’ to opt in to sharing your records.

If you have not received the text message and would like to share your record, please contact the surgery, our reception team will be able to take your consent.

Cancelling your Appointment

If you are unable to attend an appointment with one of the doctors or nurse, please telephone or use the link at the bottom of this page to cancel your appointment.

In September the total number of people who did not attend their appointment was 66 this equates to 14.5 hours of time wasted. Not attending cost NHS £160 approx. Please call the surgery on  0208 743 1511 to cancel or rearrange

By giving us as much notice as you can you are helping us to make sure that someone else is given your slot.

Test Results

Please call between 12 noon and 1.00pm only to enquire about your test results as our reception staff will have more time to deal with your request between these times and a GP is available to speak to if necessary.

New Patients

Those wishing to register must live within the practice area (see Roads within our Practice Area below) if it is not listed call the surgery for advise. Those wishing to register can download the registration forms by following the links below (All MUST be completed).Alternatively you may obtain all of the necessary forms from reception. In either case the completed Registration Enquiry, Alcohol consumption questionnaire,GMS1 form, NHS number mandatory along with proof of identity and (proof of address within last 3months) must be returned in person to reception.The registered GP will be your named GP who will have overall responsibility for the care and support that our surgery provides. Patients are normally accepted onto the list within 14 days after returning their completed forms.

An appointment with the Nurse, HCA or Doctor may then be given.

Patients who are removed from our practice list for eg. register with a new GP Practice Health Authority reason or did not attend appointment x3 will be electronically removed from our practice list. Please note ALL future appointments and alerts will automatically cancel due to the transfer.

Practice Area

The Practice area has been notified to the Hammersmith & Fulham PCT as being half a mile from the surgery by road when travelling by car but on the east side as far only as the Metropolitan Line Railway, and in the north as far as Westway.We do not accept patients living on the White City Estate.
